Consent for In-Person Treatment During COVID-19

During these unprecedented times of the COVID-19 pandemic, there is a known risk inherent in performing therapy sessions in person. It is the policy of Discovery Counseling Center to do sessions via telehealth when possible, but we understand that at times in-person sessions are required. In order to mitigate the risks, the following policies will be enforced:
  • All clients will have their temperature taken when they arrive in the office.
  • If the client has a temperature above 100 degrees or has been recently sick, the session may be rescheduled and replaced with a telehealth platform.
  • If the client may have been around someone with COVID, it is their responsibility to inform their therapist. This will require sessions to be held via telehealth rather than in person.
  • The waiting room is closed. Clients are asked to wait in their car and text/contact their therapist when they arrive. The therapist will come out and welcome you to the office.
  • Face masks must be worn at all times in the office. Clients must provide their own masks.
  • Social distancing is in place. The client and therapist will strive to maintain 6 feet distance during sessions.
  • Hand sanitizer is available for use in the office.
  • The offices are cleaned each night and counseling rooms are cleaned between clients.
 
I understand the above and will adhere to the stated requirements while in the office. I understand that sessions may be moved to telehealth with minimal notice, if necessary. I accept the known risk of in-person sessions and will not hold Discovery Counseling Center or any of its employees responsible in the case that my child becomes sick.
